A student shall NOT be permitted to practice or compete in the fall until the following items are completed....

1. Set up a Privit Account
2. Complete the MSHSAA Pre-Participation form below.  Have insurance card ready.
3. Complete the Physical Packet below.  This will require getting a physical
4. Complete SWAY Testing.  Parents, you will sign the consent form in PRIVIT and then your player will complete the testing here at Park Hill with our Athletic Trainers.
